CPE Return - Wrong and Invalid Contacts , How to Return?

Dear Sir/Madam,

With respect to termination of my account 1120725305, I received an email stating Service disconnection has been initiated along with the service provider details to return the CPE.

But the details of the service provider are wrong and invalid. How to Return now?
